Basic Life and AD&D
MAPFRE provides you with basic life insurance coverage equal to two times your annual base salary—at no cost to you. The maximum basic life insurance benefit is $400,000.
Your basic coverage also includes AD&D insurance. AD&D coverage doubles your life insurance benefit if your death is accidental and also pays benefits to you if you lose sight, speech, hearing or a limb(s) as a result of an accident.
Supplemental Life and AD&D
If eligible, you have the option to purchase supplemental life and AD&D insurance for yourself equal to between one and five times your annual base salary, up to a maximum benefit of $800,000. You must submit satisfactory Evidence of Insurability (EOI) if you:
- Are enrolled for supplemental life insurance and want to increase your coverage
- Previously waived supplemental life insurance and choose to enroll for coverage at a later date
- Are newly eligible and elect supplemental life insurance of more than $100,000
Amounts requiring EOI will not become effective until The Hartford provides written approval of your application.
Note: Life and AD&D coverage (both basic and supplemental) decreases by 50% when you reach age 70.
Dependent Life and AD&D
If eligible, you have the option to purchase life and AD&D insurance for your spouse or domestic partner (DP), and life insurance for your eligible dependent child(ren). Dependent life and AD&D insurance amounts are shown below:

EOI must be submitted if you:
- Submit a late application
