Eligibility

A leave of absence refers to your right to take time off from work for certain reasons, if you qualify. Leaves of absence are generally unpaid. For employees who are absent from work due to their own illness, wage continuation while out of work may be available through MAPFRE’s disability plans (short and/or long term disability). For employees working in certain States (such as CA, CT, MA, NY, RI, WA), Statutory benefits may also be available for their own serious health condition and/or to care for a covered family member. By answering a few simple questions, you will have an idea of what you may be eligible for and how to apply for benefits.

To initiate a Leave of Absence Claim contact the Hartford: The Hartford: 1-888-301-5615 (Policy Number 803631). After your claim is set up by phone, create an account at www.abilityadvantage.thehartford.com to manage your claim, complete the forms, submit documentation and more.
